Directory Information Form - Instructions and Sample

Holiday Island Community Church is collecting information to produce a new church directory. We are doing the directory in-house so that we can control the production and include more information. Our goal is to have a directory that will help us to get to know each other better. The Bible portrays the local body as an intimately connected group of believers, ministering to each other in the name of Christ. Help us get to know you by filling out the information form and providing a brief biography of you and yours. See example below. We realize that some people may want to share less information than others and that is just fine. We still want your picture, name, address, and phone for the directory as in the past. We are aiming at a maximum of 1/2 page per family. We will be taking our own pictures and will let you know the dates as soon as they are set. If you have a family picture that you would like us to use, we can scan it in, or you can send it electronically as a separate attachment. The best results come from unedited electronic photos.

John and Donna hail from Kewaunee, Wisconsin (east of Green Bay, south of Door County) on the shore of Lake Michigan. They are both graduates of Kewaunee High School class of 1973, and were high school sweethearts.

Donna earned a BS in Social Work from UW-Oshkosh and has worked in related fields since.

John has a BS in Theology, Masters in Biblical Counseling, and Doctorate in Biblical Studies from Master's Divinity in Evansville, Indiana. He was ordained in 2004 and has been a full time pastor since then. Before ordination, John had a computer business, published and edited a weekly newspaper, and spent ten years in the Army.

John and Donna have served churches in Illinois, Washington, and Kansas before coming to Holiday Island in April of 2011.

Donna enjoys reading, volunteer work, and family time. John enjoys reading, computers, home remodeling, fishing, and music. He is a drummer.
